    What results do patients see on Mounjaro?

    In the SURMOUNT-1 trial (New England Journal of Medicine, 2022), adults without diabetes who took tirzepatide 15mg alongside diet and activity advice lost an average of 20.9% of their body weight over 72 weeks, compared with 3.1% on placebo, counting everyone who started treatment. Among people who stayed on treatment, the average was 22.5%. On 15mg, 91% lost at least 5% of their weight, compared with 35% on placebo. Individual results vary.

    SURMOUNT-5 (2025) compared the highest dose each person could tolerate of tirzepatide (Mounjaro, 10mg or 15mg) and semaglutide (Wegovy, 1.7mg or 2.4mg) in 751 adults without diabetes who had a BMI of 30 or more, or 27 or more with a weight-related condition, with advice on a reduced-calorie diet and more physical activity. It was randomised and open-label, so people knew which medicine they were taking. Funded by Eli Lilly, which makes Mounjaro. Aronne LJ et al. N Engl J Med 2025;393:26-36.

    The four SURMOUNT weight-management trials in the UK product information included 4,838 adults. In the SURMOUNT-1 trial (New England Journal of Medicine, 2022), adults without diabetes who took tirzepatide 15mg alongside diet and activity advice lost an average of 20.9% of their body weight over 72 weeks, compared with 3.1% on placebo, counting everyone who started treatment. Among people who stayed on treatment, the average was 22.5%.

    In SURMOUNT-5 (New England Journal of Medicine, 2025), a 72-week trial comparing the two medicines directly in 751 adults with obesity but not diabetes, people on the highest dose they could tolerate of tirzepatide lost an average of 20.2% of their body weight, compared with 13.7% on semaglutide.     In SURMOUNT-1, adults on tirzepatide 15mg lost an average of 20.9% of their body weight over 72 weeks, counting everyone who started treatment, and 22.5% among those who stayed on it. Mounjaro reduces appetite, slows gastric emptying and improves insulin sensitivity by acting on both GLP-1 and GIP receptors, rather than a single hormone pathway.

    Unlike earlier GLP-1 medications that target a single hormone pathway, Mounjaro activates both GLP-1 and GIP receptors. In SURMOUNT-5, the trial that compared the two directly, people on tirzepatide lost an average of 20.2% of their body weight, compared with 13.7% on semaglutide.

    What are the Mounjaro doses and how do they increase?

    All patients start at 2.5mg once a week. The dose then steps up through 5mg, 7.5mg, 10mg and 12.5mg to a maximum of 15mg, with increases every 4 weeks under pharmacist guidance and based on how well you tolerate each step. Not everyone needs the highest dose, and your pharmacist reviews you before each increase.

    Is Mounjaro right for you?

    Mounjaro may be suitable if you are aged 18 to 85, live in the UK, and have a BMI of 30 or above, or a BMI of 27 to 29.9 alongside a listed weight-related condition such as high blood pressure or prediabetes. It is not suitable if you have type 1 diabetes, a history of pancreatitis, a personal or family history of medullary thyroid carcinoma or MEN 2, or if you are pregnant.

    What are the side effects of Mounjaro?

    Most Mounjaro side effects are gut related and improve as your body adjusts. Nausea, diarrhoea, vomiting, constipation and decreased appetite affect more than 1 in 10 people. Indigestion, abdominal pain, injection site reactions, fatigue and dizziness are common. Starting at the lowest 2.5mg dose and increasing gradually reduces how severe they are, and most people find they improve substantially after the first 4 to 8 weeks.

    Managing them

    Starting at the lowest 2.5mg dose and increasing gradually significantly reduces the severity of side effects. Eating smaller meals and staying hydrated helps. Most people find side effects improve substantially after the first 4 to 8 weeks.

    Practical guidance

    How do you inject Mounjaro?

    Use a new needle for every injection and rotate between your abdomen and thigh. The back of your upper arm can also be used, but someone else should give that injection for you. Inject on the same day each week. If you need to move your injection day, you can, as long as at least 3 days (72 hours) pass between two doses. Then inject on your new day each week. Keep unopened pens in the fridge. Once in use, Mounjaro can stay out of the fridge for up to 30 days, not above 30 °C.

    Contraception (Mounjaro only): tirzepatide can reduce absorption of the contraceptive pill. Use a non-oral method or add condoms for 4 weeks after starting and for 4 weeks after each dose increase. Wegovy needs no extra precautions unless severe vomiting or diarrhoea occurs, then follow missed-pill guidance.

    Pregnancy: these medicines must not be used in pregnancy. Stop and contact us or your GP immediately if you become pregnant. If planning pregnancy: stop Mounjaro at least 1 month before, Wegovy at least 2 months before.

    Seek advice promptly if you notice: signs of dehydration during vomiting or diarrhoea (dizziness, reduced urination, dry mouth): keep sipping fluids, as dehydration can affect your kidneys; palpitations or a racing heartbeat at rest; a new neck lump, swelling, or hoarseness. If you have type 2 diabetes and use insulin or a sulfonylurea, know the signs of low blood sugar (sweating, shaking, sudden hunger, confusion) and treat promptly.

    Before any operation: tell your surgical or anaesthetic team you take a GLP-1 medicine. It slows stomach emptying, which matters for anaesthesia.
