Can You Take Mounjaro a Day Early (or Late)? Moving Your Injection Day

Yes, if at least 3 days (72 hours) pass between doses. A UK pharmacist explains early and late Mounjaro doses, missed doses and moving your injection day.

Short answer: yes. You can take Mounjaro up to 4 days early, as long as at least 3 days (72 hours) pass between two doses, so a day or two early is fine. Then inject on that new day each week. You can take it late too: if fewer than 4 days have passed since your usual day, take it as soon as you remember; if 4 days or more have passed, skip it and inject on your usual day. Never take two doses to make up for a missed one.

These are the rules in the UK licence for Mounjaro (its Summary of Product Characteristics, section 4.2, as of October 2026), and they are the ones your pharmacist follows. This guide works through each case with a usual injection day of Sunday, so you can see exactly which days work.

    Our pharmacist's advice

Inject on the same day each week. If you need to move your injection day, you can, as long as at least 3 days (72 hours) pass between two doses. Then inject on your new day each week.

Missed dose. Mounjaro: take it if fewer than 4 days have passed, otherwise skip. Wegovy pens: take it if fewer than 5 days have passed, otherwise skip. After a long gap, contact us before restarting: treatment resumes at the lowest dose.

    The two numbers to remember

    • Shortest gap between doses72 hours

If you need to move your injection day, you can, as long as at least 3 days (72 hours) pass between two doses.

    • Late dose window4 days

Mounjaro: take it if fewer than 4 days have passed, otherwise skip. The days count from your usual day.

Everything else in this guide follows from those two rules. Mounjaro can be injected at any time of day, with or without meals, so the time of day only matters when you are counting.

Can you take Mounjaro a day early?

Yes. Say you usually inject on Sunday and want to inject on Saturday instead. Your last dose was 6 days earlier, well over the 3 days (72 hours) the licence asks for between two doses, so you can. From then on, Saturday is your new day. Then inject on your new day each week.

Would you rather go back to Sunday afterwards? You can. That is one day later than your new day, which leaves 8 days between doses, and a dose a day late is within the rules.

Can you take Mounjaro 2 days early?

Yes. Two days early, on Friday if you usually inject on Sunday, leaves 5 days since your last dose. That is more than 3 days, so it is within the rule. Then carry on with Friday as your injection day each week.

Can you take Mounjaro 3 or more days early?

Three days early is fine: Thursday leaves 4 days since a Sunday dose. Four days early is the limit: Wednesday leaves exactly 3 days, so only inject once a full 72 hours have passed, counted to the hour. If your last dose was at 8pm on Sunday, the earliest you can inject is 8pm on Wednesday. Five or six days early, on Tuesday or Monday, is too soon.

Your last dose was on Sunday. You inject onTime since your last doseCan you?

    Thursday (3 days early)4 daysYes

Tuesday (5 days early)2 daysNo, too soon
Monday (6 days early)1 dayNo, too soon

Can you take Mounjaro early every week?

No. Moving your day is a one-off. Mounjaro is a once-weekly medicine, and the leaflet says that after you choose a new day, you carry on once a week on that day. Injecting every 5 or 6 days would mean more doses than prescribed.

Each KwikPen holds 4 weekly doses. A little medicine may be left in the pen after the 4th dose, and the leaflet says not to try to use it. Our guide to the Mounjaro "golden dose" explains why.

Can you take Mounjaro a day late?

Yes. Mounjaro: take it if fewer than 4 days have passed, otherwise skip. A day late, on Monday if you usually inject on Sunday, is well within that. Take it as soon as you remember, then take your next dose on your usual day, as the leaflet says.

Because a late dose is always taken fewer than 4 days after your usual day, your next dose on your usual day is always more than 3 days later, so the two rules never clash.

What if you are several days late?

In practice, for Mounjaro, counting from your usual day:

    • If fewer than 4 days have passedTake it as soon as you remember, then take your next dose on your usual day.
    • If 4 days or more have passedSkip it and take your next dose on your usual day.

Never take two doses to make up for a missed one.

You missed Sunday. It is nowDays since your usual dayWhat to do
Monday1Take it now. Next dose on Sunday
Tuesday2Take it now. Next dose on Sunday
Wednesday3Take it now. Next dose on Sunday
Thursday4Skip it. Next dose on Sunday
Friday or Saturday5 or 6Skip it. Next dose on Sunday

The days count from the time you would usually have injected. If you usually inject on Sunday evening and it is Thursday morning, fewer than 4 days have passed. When it is that close, message your pharmacist before you inject.

What if you missed a whole week, or more?

Missed one dose completely, and it is your usual day again? Take this week's dose as normal. The missed dose is skipped, never made up.

Missed two or more doses in a row? That is a gap in treatment rather than a missed dose. Message your pharmacist before your next injection and tell them the date of your last dose, because how long the gap was decides the dose you restart on. Our guide to restarting Mounjaro or Wegovy after a break explains how.

Thinking of starting Mounjaro? Every order is reviewed by a GPhC-registered pharmacist, and you can message your pharmacist from your account whenever your injection day needs to move.

How do you change your Mounjaro injection day for good?

    1. Choose your new day. Pick a day you can keep to every week. Any time of day works, with or without meals.

    2. Count from your last injection. Your first dose on the new day must be at least 3 days (72 hours) after your last one.

    3. Moving to a day just after your usual one? If your new day is 1 to 3 days after your usual day (Sunday to Tuesday, say), skip your usual day that week and inject on the new day instead. That is the same as a dose 1 to 3 days late, which is within the rules.

    4. Keep to it. Then inject on your new day each week. Lilly's instructions for the pen have a calendar page where you can write down the day you choose.

Moving from Sunday toYour first dose on the new day
Monday or TuesdaySkip your next Sunday and inject on the Monday or Tuesday after it, 8 or 9 days after your last dose
WednesdayThe Wednesday straight after your last dose, once 72 hours have passed, or skip your next Sunday and inject on the Wednesday after it, 10 days after your last dose
Thursday, Friday or SaturdayThe Thursday, Friday or Saturday straight after your last dose, 4, 5 or 6 days after it

What happens to your pen when your day moves?

    Mounjaro KwikPen 2.5mg
The Mounjaro KwikPen holds 4 weekly doses.

Lilly's instructions say to dispose of the pen after 4 weekly doses, or 30 days after you first used it, whichever comes first, even if some medicine is left. Moving your day does not change that.

    • Moving earlier means your pen runs out a few days sooner than planned.
    • Skipping a dose, or moving later, can take your 4th dose past day 30. If it does, put the pen in your sharps bin anyway and message your pharmacist, so you are not left short.

If a change of day means you will run out before your next order arrives, message your pharmacist. Our help centre explains what to do if you need your next order sooner.

What about travelling across time zones?

The Mounjaro licence and leaflet say nothing about time zones, so there is no separate rule. The same two rules apply: at least 3 days (72 hours) between two doses, and fewer than 4 days after your usual day for a late dose. Count the hours since your last injection, not the dates. A time zone change moves the count by hours, so it only matters when you are already close to one of those lines. If you are not sure, message your pharmacist before you travel, with your flight times and the time you usually inject.

    • If your injection day falls on a travel day, you can move it a day or two earlier before you go, as above.
    • Carry your pens in your hand luggage, never in the hold, where they could freeze.
    • Keep your pharmacy label with your pens. Solira patients can ask for a travel letter for an order from their account, signed by their pharmacist.

    Our pharmacist's storage advice

Storage: keep unopened pens in the fridge (2–8 °C) and never use a pen that has been frozen. Once in use: Mounjaro can stay out of the fridge for up to 30 days, not above 30 °C (then discard, even if medicine remains); Wegovy pens from 0.25 mg to 2.4 mg keep for 6 weeks below 30 °C with the cap on. The Wegovy 7.2 mg single-dose pen can be kept out of the fridge for up to 28 days, not above 30 °C; throw it away after that.

More on keeping pens cold while you are away in our guide to Mounjaro storage, the fridge and travelling.

What if two doses end up too close together?

If you have injected twice within 3 days, for example because you forgot you had already taken this week's dose, do not inject again. Message your pharmacist straight away, and wait for them to tell you when to take your next dose. NHS advice is to get help from NHS 111 if you have used more than your prescribed dose.

The Mounjaro leaflet says too much of the medicine may cause low blood sugar and can make you feel sick or be sick. Low blood sugar is more likely if you also use insulin or a sulfonylurea for type 2 diabetes. Tirzepatide, the medicine in Mounjaro, leaves the body slowly (its half-life is about 5 days), so the licence says these symptoms may need watching and treating for a prolonged period.

See also how to inject Mounjaro, the Mounjaro dosage chart and when to increase your Mounjaro dose.

When should you speak to your pharmacist?

Message your pharmacist before you inject if you are not sure when your last dose was, if you are close to the 72-hour or the 4-day line, if you have missed two or more doses in a row, or if you are crossing time zones and not sure how to count.

Message your pharmacist too if your pen reaches 30 days with a dose still in it, or if you will run out before your next order arrives.

If you have injected twice within 3 days, or used more than your prescribed dose, do not inject again and message your pharmacist straight away. NHS advice is to get help from NHS 111 if you have used more than your prescribed dose.

Stop your treatment and seek urgent medical help if you experience: severe, persistent stomach pain, especially spreading to your back, with or without vomiting. This can be a sign of pancreatitis. Sudden pain in the upper right of your tummy, fever, or yellowing of the skin or eyes: possible gallbladder problems. Sudden loss of vision, or loss of part of your field of vision: seek same-day eye assessment. Swelling of the face or lips, difficulty breathing, or a widespread rash: call 999.

Seek advice promptly if you notice: signs of dehydration during vomiting or diarrhoea (dizziness, reduced urination, dry mouth): keep sipping fluids, as dehydration can affect your kidneys; palpitations or a racing heartbeat at rest; a new neck lump, swelling, or hoarseness. If you have type 2 diabetes and use insulin or a sulfonylurea, know the signs of low blood sugar (sweating, shaking, sudden hunger, confusion) and treat promptly.

Before any operation: tell your surgical or anaesthetic team you take a GLP-1 medicine. It slows stomach emptying, which matters for anaesthesia.

For urgent medical concerns, call NHS 111 or 999. See when to get urgent help.
