    Blocking fat absorption at source

    Orlistat is a lipase inhibitor. It works in the gut to block the enzyme that breaks down dietary fat. Approximately one third of the fat you eat is prevented from being absorbed and is instead passed out of the body. Unlike GLP-1 medications, orlistat does not affect appetite or the brain. It works directly in the digestive system to reduce calorie absorption from fat.

    Orlistat 120mg has been licensed in the UK since 1998. The pivotal XENDOS trial (Torgerson et al. Diabetes Care, 2004) followed 3,305 patients over 4 years and demonstrated that orlistat combined with lifestyle modification produced significantly greater weight loss than lifestyle modification alone, and reduced the risk of developing type 2 diabetes by 37.3% in patients with impaired glucose tolerance. NICE recommends orlistat as a first-line adjunct to diet and exercise for patients with a BMI ≥28 with risk factors, or BMI ≥30.

    What you need to know before starting orlistat

    Diet is essential

    Orlistat works alongside a calorie-controlled diet with no more than 30% of calories from fat. Without dietary change, side effects increase significantly.

    Take a multivitamin

    Orlistat reduces absorption of fat-soluble vitamins (A, D, E, K). Take a daily multivitamin at least 2 hours after your orlistat dose or at bedtime.

    Side effects are manageable

    Oily stools, flatulence, and urgency are common, particularly if dietary fat intake is high. Following a low-fat diet minimises these effects significantly.

    12-week review

    Treatment is reviewed at 12 weeks. If you have not lost at least 5% of your starting body weight, treatment should be discontinued.

    What to expect

    Most side effects of orlistat are gastrointestinal and directly related to the mechanism of action. They occur when dietary fat intake is high. Following the recommended low-fat diet substantially reduces their likelihood and severity.

    Common questions

    Alli contains orlistat 60mg and Orlos is a branded version of orlistat 60mg. They are therapeutically identical. Our 60mg capsules are the same active ingredient at the same dose.

    Orlistat works immediately, from the first dose it begins blocking approximately one third of dietary fat absorption. Weight loss typically becomes visible within 2 to 4 weeks when combined with a calorie-controlled diet.

    Some medications interact with orlistat. Our pharmacist will review your medication list during the consultation. Key interactions include ciclosporin, amiodarone, warfarin, levothyroxine, and some epilepsy medications.

    A calorie-controlled diet with no more than 30% of calories from fat. Spreading fat intake evenly across meals helps minimise gastrointestinal side effects. Your pharmacist can provide dietary guidance.

    Treatment is reviewed at 12 weeks. If you have lost at least 5% of your starting body weight, treatment can be continued. If not, it should be discontinued and alternative options considered.
