The science
DHT, the hormone behind male pattern hair loss
Dihydrotestosterone (DHT) is derived from testosterone by the enzyme 5-alpha reductase. In genetically susceptible men, DHT causes hair follicles to gradually miniaturise, producing progressively finer hairs until growth stops entirely. Finasteride blocks 5-alpha reductase, reducing scalp DHT by approximately 60% to 70%. Without the DHT signal, miniaturisation slows or stops and existing follicles begin to recover.

Finasteride 1mg has been licensed in the UK since 1997. The PLESS trial and 5-year extension showed 83% on finasteride had no further hair loss at 2 years versus 72% continuing to lose hair on placebo. At 5 years, 90% maintained or improved their hair count. Men who stopped treatment saw reversal within 12 months.

Most men notice reduced shedding within 3 months. Visible regrowth typically becomes apparent after 6 to 12 months of consistent daily use.
Within 6 to 12 months of stopping, the protective effect is lost and hair loss typically resumes at the rate it would have without treatment. Regrowth achieved on treatment will gradually be lost.
