Short answer: under three months since your last dose, you can usually carry on at the dose you were on, once your pharmacist has checked it. If you reorder from us, the reorder suggests the next step up with your last dose still there to choose, except for Wegovy injections more than four weeks after your last supply ran out, where it suggests your last dose again. Three months or more, you restart from the lowest dose and build up again: 2.5mg for Mounjaro, 0.25mg for Wegovy injections, 1.5mg for Wegovy tablets and 0.8mg for Foundayo. The Mounjaro and Wegovy licences do not set a restart dose after a break, so this is the rule your pharmacist at Solira follows.
This guide explains why the gap matters, exactly where you restart in each situation, and what to do before your first dose back.
Is it a missed dose or a break?
One missed dose is not a break. The licences say what to do, and you carry on with your usual schedule afterwards:
- Injection pens. Missed dose. Mounjaro: take it if fewer than 4 days have passed, otherwise skip. Wegovy pens: take it if fewer than 5 days have passed, otherwise skip.
- Wegovy tablets. Missed a dose? Skip it and take your next tablet the following day. Never double up.
- Foundayo. Missed a dose? Take it as soon as you remember on the same day. If you only remember the next day, skip it. Never take two tablets in one day.
A break is longer: weeks or months without treatment, because you paused, ran out or stopped. That is when your restart dose matters, and the first thing to do is message your pharmacist.
Why does the restart dose depend on how long you stopped?
Every one of these medicines starts at a low dose and steps up gradually. The reason is stomach side effects. In the Mounjaro licence, nausea, diarrhoea and vomiting happened mostly while the dose was being increased and became less common over time. The Wegovy licence steps the dose up over 16 weeks to make stomach symptoms less likely.
After a long break, going straight back to a high dose would skip those steps. The Wegovy licence says that when more doses are missed, a lower starting dose should be considered when treatment restarts. That is why, for Wegovy injections, our reorder suggests your last dose again rather than a step up after more than four weeks off. So the first thing your pharmacist needs to know is the date of your last dose.
What dose do you restart on after 3 weeks, a month or 3 months?
| Time since your last dose | Ordered from Solira before (the reorder) | New to Solira (the consultation) |
|---|---|---|
| Four weeks or less | The next step up is suggested, and your last dose is still there to choose | Usually your last dose, once your pharmacist has checked your proof |
| More than four weeks, under three months | Mounjaro, Wegovy tablets and Foundayo: the next step up is suggested, and your last dose is still there to choose. Wegovy injections: your last dose is suggested again, and the next step up is one tap away | Usually your last dose, once your pharmacist has checked your proof |
| Three months or more | The lowest dose, then up one step at a time | The lowest dose, then up one step at a time |
Your pharmacist reviews every order before anything is sent.

How do you restart if you ordered from Solira before?
Reorder from your account. The four-step reorder works out how long it has been since your last supply ran out and offers the dose that fits:
- Under three months: the reorder suggests the next step up from your last dose, and your last dose is still there to choose. Your pharmacist reviews every order before anything is sent.
- The exception, Wegovy injections more than four weeks after your last supply ran out: the reorder suggests your last dose again, and the next step up is one tap away.
- Three months or more: the lowest dose, and you build up again.
Planning a break? Choose Take a break in your account and pick the date you would like a reminder. It shows the date of your last dose and reminds you: "A break of three months or more after it means restarting from the lowest dose."
How do you restart if you had treatment somewhere else?
Start the online consultation and say that you are restarting after a break. You will be asked which medicine you took, when you last used it and your last dose.
- Under three months: you can usually carry on with the same medicine at the dose you were on, once your pharmacist has checked your proof. This applies to Mounjaro, Wegovy, Wegovy tablets and Foundayo. Any one of these: an approval email from your previous provider that shows your name, the date of the email and the medication you were approved for; a photo of your prescription from your previous provider; or a photo of the dispensing label clearly showing the medicine, the dose, your name and the date. You can upload it during the consultation, or after you pay.
- Any medicine, three months or more: the lowest dose.
Still being supplied by another provider, with no break? That is a switch rather than a restart: see switching to Solira from another provider.
What should you check before your first dose back?
- The contraceptive pill. The advice for starting treatment applies again. Contraception (Mounjaro only): tirzepatide can reduce absorption of the contraceptive pill. Use a non-oral method or add condoms for 4 weeks after starting and for 4 weeks after each dose increase. Wegovy needs no extra precautions unless severe vomiting or diarrhoea occurs, then follow missed-pill guidance. For Foundayo: If you use the oral contraceptive pill, use a non-oral method or add a barrier method such as condoms for 30 days after starting treatment and for 30 days after each dose increase.
- Type 2 diabetes. If you use insulin or a sulfonylurea, tell your pharmacist before you restart.
- One supply at a time. Never take GLP-1 medicines from two places, or two GLP-1 medicines, at the same time.
Thinking about taking a break?
Treatment works alongside a reduced-calorie diet and regular activity. Aim for at least 150 minutes a week. Weight loss naturally slows over time, and stopping treatment without keeping lifestyle changes commonly leads to weight regain, so we review you regularly and plan any stop together.
Thinking of stopping because of side effects? Speak to your pharmacist first. And remember the rule above: a break of three months or more means restarting from the lowest dose.
