Mounjaro (tirzepatide) doesn't stop working the moment you stop taking it. Understanding how long it stays in your system matters for stopping, switching, planning a pregnancy, and going into surgery.
Half-life and clearance
Tirzepatide has a half-life of approximately five days, which is why it's dosed once weekly. Semaglutide (the active ingredient in Wegovy and Ozempic) has a half-life of around a week. As a rule of thumb, it takes about five half-lives for a drug to be effectively eliminated, so roughly four to five weeks for tirzepatide. For semaglutide, the Wegovy product information says it stays in the blood for about 7 weeks after the last 2.4mg injection, and about 5 to 7 weeks after the last 25mg tablet.

Why this matters practically
- Side effects don't stop the day you stop. Nausea, reflux and appetite suppression taper off over the weeks after your last dose as blood levels fall.
- Appetite returns gradually rather than overnight. Most people notice hunger and food noise creeping back over three to five weeks, not immediately.
- Pregnancy planning. The UK product information says to stop Mounjaro at least 1 month before a planned pregnancy, and Wegovy at least 2 months before. Keep using contraception until that time has passed, and plan the stop with your prescriber. Do not stop and try to conceive immediately.
- Switching between GLP-1 medicines. Because the previous drug is still in your system for weeks, switching is not a like-for-like swap and needs prescriber input.
- Surgery. Tell your anaesthetist you take a GLP-1 medicine. Delayed gastric emptying affects pre-operative fasting guidance and there is specific anaesthetic guidance on GLP-1 receptor agonists.
What affects how quickly it clears
- The dose you were on, higher doses take longer to fall below active levels.
- How long you've been treated, steady-state levels build up over the first weeks.
Kidney and liver function do not affect it. The UK product information says kidney or liver problems do not change how the body clears tirzepatide. Tell us about any kidney or liver condition all the same, as it still matters for whether Mounjaro is suitable for you.
A useful mental model: after your last dose, expect roughly a month before you can consider the drug "gone", and expect appetite to return gradually across that window, not overnight.
