Week 1. Starting the routine
You take your first 1.5mg tablet first thing in the morning, on an empty stomach, with up to 120ml of plain water, then wait 30 minutes before eating or drinking anything else. Nothing dramatic happens on day one. The medicine is building up in the background.
In this first week, most people notice mild nausea in the first day or two, and possibly a couple of days of low-level indigestion or fullness. Appetite may already feel a little quieter by the end of the week, meals feel smaller, snacking pulls less.
Week 2, The changes settle in
Early side effects (nausea, indigestion) usually ease through week two. On average, the first bout of nausea lasts around 13 days, and vomiting. If it happens at all, around two days. If yours is still going strong at the end of week two, message your pharmacist rather than pushing through.
Appetite change is now clearer. People often describe it as forgetting to be hungry, or feeling full three or four bites earlier than usual. This isn't willpower; it's the medicine doing what it's designed to do.
Week 3. Habits start to shift
By week three the routine feels normal. Constipation can build gradually in this stage, affecting around one in five people, because everything through the gut moves more slowly. Fluids, fibre and gentle movement usually manage it. Occasionally a mild laxative helps short term; ask your pharmacist which are safe.
Weight on the scales may still be quiet. Some people see a small drop; some don't. The first month is more about setting the routine than seeing a number.
Week 4. Preparing for the next step
As the month ends, your pharmacist reviews you before the step up to 4mg. If week three felt stable, you're likely ready. If side effects were still difficult, staying another month at 1.5mg is a normal choice, not a setback.
Meaningful, steady weight loss usually starts to build from month two onwards, as the dose rises. That's the pattern to plan around: month one is the foundation. Managing common issues
- Nausea: smaller, blander meals; avoid greasy or very rich food; sip water between meals rather than with them.
- Constipation: aim for 1.5 to 2 litres of fluid a day, add fibre gradually, walk after meals.
- Fatigue: often reflects lower calorie intake; check you're eating enough protein.
- Indigestion: avoid lying down soon after meals; smaller meals help.
Red flags. Stop and get reviewed
- Severe or persistent vomiting you can't control with hydration.
- Severe abdominal pain, especially if it radiates to the back, possible pancreatitis.
- Sudden vision changes.
- A new lump in the neck, hoarseness or trouble swallowing.
- Signs of dehydration you can't reverse (dark urine, dizziness, headaches).
Frequently asked questions
When will I notice appetite change? Usually within the first one to two weeks. Meals feel smaller, and the pull toward snacking softens. It's a quiet change, not dramatic.
How long does the early nausea last? Around 13 days on average, according to OASIS-4 data, often less. It's most noticeable in the first week after starting or stepping up.
When will I see weight loss on the scales? Small movement in month one is possible but not guaranteed. Meaningful, steady loss usually starts to build from month two onwards as the dose titrates up.
Should I follow a special diet? No prescribed diet. Prioritise protein, hydrate well, and eat smaller portions, appetite will help you get there naturally.
