Coming Off Wegovy: How Long to Take It and What Happens When You Stop

How long to take Wegovy, whether you can just stop, what happens to appetite and weight afterwards (STEP 1 and STEP 4), keeping weight off and restarting.

Short answer: Wegovy (semaglutide) has no fixed length of treatment in its UK product information, which says a decision is needed on whether to carry on if you have not lost at least 5% of your starting weight after 6 months (SmPC section 4.2), and we review you regularly and plan any stop with you. You can come off Wegovy at any time, but talk to your pharmacist first, and if you have type 2 diabetes the NHS says stopping could affect your blood sugar. Semaglutide stays in the circulation for about 7 weeks after the last 2.4mg dose (SmPC section 5.2), so its effect on appetite wears off over the following weeks. Weight regain is common: in the STEP 1 trial extension (Diabetes, Obesity and Metabolism, 2022), which followed 327 adults for a year after they completed 68 weeks of treatment, those who had taken semaglutide 2.4mg had lost 17.3% of their weight on average, then regained 11.6 percentage points, ending 5.6% below their starting weight. Individual results vary.

This guide sets out what the UK product information, the NHS and the two Wegovy trials that stopped treatment say, as of October 2026, for the weekly pens and for Wegovy tablets. A GPhC-registered pharmacist reviews every Solira consultation and reorder before anything is supplied.

How long do you take Wegovy for weight loss?

There is no fixed length: the UK product information for Wegovy sets no end date for treatment, and asks for a decision on whether to continue if you have not lost at least 5% of your starting weight after 6 months on treatment (SmPC section 4.2, revised September 2026). For someone who starts at 100kg, 5% is 5kg. NICE's guidance for the NHS also says to consider stopping semaglutide if less than 5% of the starting weight has been lost after 6 months (TA875, recommendation 1.2).

The first 16 weeks are spent building up the dose, from 0.25mg to the 2.4mg maintenance dose, to make stomach side effects less likely (SmPC section 4.2). Every step is in our Wegovy dose schedule, and what to expect week by week is in how long Wegovy takes to work.

The safety data in the Wegovy product information include the SELECT trial, in which 8,803 adults with established cardiovascular disease took Wegovy for a median of 37.3 months (SmPC section 4.8). How long any one person takes it is a decision made with them and reviewed over time. This is how we decide:

We'll pause or stop treatment if it isn't working for you (less than 5% weight loss after a fair trial at your best-tolerated dose), if side effects persist despite adjusting, if your BMI falls below 20, or if you become pregnant. Treatment is reviewed regularly, and if longer-term therapy beyond our service is right for you, we'll help you transition to a prescriber-led pathway.

Can you just stop taking Wegovy?

Yes, you can stop Wegovy at any time, but speak to your pharmacist first, so the stop is planned and you know what to expect. The NHS advises talking to your doctor if you want to stop semaglutide, and not stopping suddenly; it adds that if you have type 2 diabetes stopping could affect your blood sugar, and that your doctor may suggest reducing your dose gradually (nhs.uk, semaglutide, reviewed 15 May 2026).

The UK product information gives no step-down schedule for stopping, so whether you stop after a last dose or come down a step first is something your pharmacist decides with you. If a side effect is the reason you want to stop, tell us first: the product information says that with significant stomach symptoms, the next dose increase can be delayed or the dose lowered to the previous one until the symptoms improve (SmPC section 4.2). Stopping is still your choice.

What happens when you stop Wegovy?

When you stop Wegovy, the medicine leaves your body slowly over about 7 weeks, its effect on appetite wears off, and hunger and weight usually start to come back. Semaglutide has a half-life of about a week, so about half of it is gone a week after your last dose, and the UK product information says it is present in the circulation for about 7 weeks after the last 2.4mg dose (SmPC section 5.2). Because the medicine fades out over weeks rather than stopping overnight, any side effects you had on treatment would be expected to ease as the level falls, rather than stop the day you stop. The changes in your body seen during treatment tend to move back as well: in the STEP 1 trial extension (Wilding and colleagues, Diabetes, Obesity and Metabolism, 2022), the improvements in heart and metabolic risk measures seen during 68 weeks of semaglutide had moved back towards where they started, for most of the measures, a year after stopping.

Will you put weight back on after Wegovy?

Weight regain is common after stopping Wegovy, but in both trials that looked at it, average weight was still below where it started at the end of the time studied. Two trials stopped semaglutide 2.4mg on purpose and followed what happened next:

    What happened after stopping?STEP 1 extensionSTEP 4
Who took part?327 adults from STEP 1 who had completed its 68 weeks of treatment, on semaglutide 2.4mg or placebo, followed for a year after both were stopped803 adults who reached semaglutide 2.4mg in a 20-week run-in, then were randomised, 2 to 1, to carry on or switch to placebo for 48 weeks
What was compared?Weight a year after stopping, for those who had taken semaglutide against those who had taken placebo, with no structured diet and activity programme in that yearCarrying on with semaglutide against switching to placebo, with a reduced-calorie diet and more activity in both groups
What happened to weight?Semaglutide group: lost 17.3% by week 68, regained 11.6 percentage points by week 120, ending 5.6% below their starting weight. Placebo group: ended 0.1% belowSwitched to placebo: weight rose 6.9% from week 20 to week 68, though on average it stayed below where it was at the start of the run-in. Carried on: a further 7.9% fall
Heart and metabolic measures? Most moved back towards where they started by week 120Waist size and systolic blood pressure improved more with continued semaglutide than with placebo
Where was it published?Wilding and colleagues, Diabetes, Obesity and Metabolism, 2022 (exploratory analyses)Rubino and colleagues, JAMA, 2021

These were separate trials in different groups of people, so the figures are not a direct comparison. Individual results vary.

In the STEP 1 extension (Wilding and colleagues, Diabetes, Obesity and Metabolism, 2022), counting the people who had completed the 68 weeks of treatment, those on semaglutide 2.4mg had lost 17.3% of their weight on average by week 68. A year after stopping, they had regained 11.6 percentage points, ending 5.6% below their starting weight, against 0.1% below for those who had taken placebo. Individual results vary. That year had no structured diet and activity programme (SmPC section 5.1), so it does not show what one would have changed.

In STEP 4 (Rubino and colleagues, JAMA, 2021), 803 adults had lost 10.6% of their weight on average during a 20-week run-in on semaglutide. Counting everyone randomised at week 20, those switched to placebo regained 6.9% of their body weight over the next 48 weeks, while those who carried on lost a further 7.9%. Individual results vary. The product information describes the weight of the placebo group as rising steadily over those 48 weeks (SmPC section 5.1).

Both trials point the same way: on average, weight comes back after semaglutide is stopped, at different speeds for different people. That is a reason to plan the stop and keep the habits that help, not a reason to stay on treatment you do not want.

Are there withdrawal symptoms when you stop Wegovy?

The UK product information for Wegovy pens and Wegovy tablets does not list any withdrawal symptoms or withdrawal reaction after stopping, as of October 2026. What people usually notice is their appetite returning as semaglutide leaves the body, which takes about 7 weeks after a last 2.4mg injection and about 5 to 7 weeks after a last 25mg tablet (SmPC section 5.2 of each).

If you have type 2 diabetes, the NHS says stopping semaglutide could affect your blood sugar, so speak to the doctor or nurse who looks after your diabetes before you stop. If you feel unwell after stopping, tell your pharmacist or GP rather than putting it down to the medicine wearing off. The side effects of treatment itself, and how often each happens, are in Wegovy side effects and is Wegovy safe?

How do you come off Wegovy?

Come off Wegovy by planning it with your pharmacist: there is no step-down schedule in the UK product information, so the plan is made around you. These are the steps we follow with patients who want to stop:

    1. Tell your pharmacist. Message us through your account before you stop, and say why. A side effect, a planned pregnancy, an operation or reaching your goal each comes with its own advice.

    2. Agree how to stop. Your pharmacist may suggest stopping after your last dose, or coming down a step first, as the NHS notes a doctor may. Do not change your dose on your own.

    3. If you have type 2 diabetes, tell your diabetes team. The NHS says stopping semaglutide could affect your blood sugar, so the doctor or nurse who looks after your diabetes should know before you stop.

    4. Keep the habits going. Keep up the eating and activity changes you made on treatment, and weigh yourself regularly so you notice a change early.

How do you keep weight off after Wegovy?

Keep the eating and activity habits you built during treatment: that is the counselling we give every patient, before and after a stop.

Treatment works alongside a reduced-calorie diet and regular activity. Aim for at least 150 minutes a week. Weight loss naturally slows over time, and stopping treatment without keeping lifestyle changes commonly leads to weight regain, so we review you regularly and plan any stop together.

The NHS physical activity guidelines for adults ask for at least 150 minutes of moderate activity a week, or 75 minutes of vigorous activity, and strengthening activities that work all the major muscles on at least 2 days a week. For food, our guide what to eat on Wegovy explains why we suggest some protein at every meal, eaten first, and how to build regular meals that keep you full.

    • Protein at every meal Fish, eggs, chicken, yoghurt, beans or lentils, eaten first, with regular meals rather than long gaps.

    • 150 minutes a week Moderate activity such as brisk walking, spread across the week, or 75 minutes of vigorous activity.

    • Strength on 2 days a week Activities that work the legs, hips, back, tummy, chest, shoulders and arms.

    • Keep weighing yourself A regular check, on the same scales, helps you notice a change early and act on it.

Should you stop Wegovy before a pregnancy or an operation?

    Before a pregnancy

Yes for a planned pregnancy: Wegovy is stopped at least 2 months before you start trying, because semaglutide leaves the body slowly (SmPC section 4.6, for the pens and the tablets alike). This is the counselling we give:

Pregnancy: these medicines must not be used in pregnancy. Stop and contact us or your GP immediately if you become pregnant. If planning pregnancy: stop Mounjaro at least 1 month before, Wegovy at least 2 months before.

Contraception while you are on treatment, and planning a pregnancy, are covered in GLP-1 medicines, contraception and pregnancy.

    Before an operation

For an operation, do not stop Wegovy on your own. This is the advice we give:

Before any operation: tell your surgical or anaesthetic team you take a GLP-1 medicine. It slows stomach emptying, which matters for anaesthesia.

Keep taking the medicine you already have as usual, unless your surgical or anaesthetic team tells you otherwise.

Can you restart Wegovy after stopping?

Yes, you can restart Wegovy, and the dose depends on how long the break was. If your last dose was under three months ago, you can usually carry on at the same dose once your pharmacist has checked it; if your last supply came from another provider, we ask for proof of it. After a break of three months or more, you restart at the lowest dose, 0.25mg for the pens or 1.5mg for the tablets, and build up again.

The product information says that when more doses are missed, a lower starting dose should be considered for restarting (SmPC section 4.2), and your pharmacist reviews your dose before anything is sent. What counts as proof, and how a restart works for each medicine, is in restarting Mounjaro or Wegovy after a break.

How do you come off Wegovy tablets?

Come off Wegovy tablets the same way as the pens: talk to your pharmacist first, and expect your appetite to return as the medicine leaves your body, which takes about 5 to 7 weeks after the last 25mg tablet (Wegovy tablets SmPC section 5.2, revised June 2026). Like the pens, the tablets are stopped at least 2 months before a planned pregnancy (SmPC section 4.6), and the NHS advice on talking to your doctor first and on type 2 diabetes applies to semaglutide tablets too.

If it is the daily routine you want to stop rather than the medicine, ask your pharmacist about swapping forms: someone on 25mg tablets can start the 2.4mg injection the day after their last tablet, and someone on the 2.4mg injection can start 25mg tablets one week after their last injection, though the product information says the effect of switching cannot easily be predicted (SmPC section 4.2 of each). The monthly steps are in our Wegovy tablets dosage schedule, and the two forms are compared in Wegovy pill vs injection.

When should you speak to your pharmacist or doctor?

Stop your treatment and seek urgent medical help if you experience: severe, persistent stomach pain, especially spreading to your back, with or without vomiting. This can be a sign of pancreatitis. Sudden pain in the upper right of your tummy, fever, or yellowing of the skin or eyes: possible gallbladder problems. Sudden loss of vision, or loss of part of your field of vision: seek same-day eye assessment. Swelling of the face or lips, difficulty breathing, or a widespread rash: call 999.

Seek advice promptly if you notice: signs of dehydration during vomiting or diarrhoea (dizziness, reduced urination, dry mouth): keep sipping fluids, as dehydration can affect your kidneys; palpitations or a racing heartbeat at rest; a new neck lump, swelling, or hoarseness. If you have type 2 diabetes and use insulin or a sulfonylurea, know the signs of low blood sugar (sweating, shaking, sudden hunger, confusion) and treat promptly.

Before any operation: tell your surgical or anaesthetic team you take a GLP-1 medicine. It slows stomach emptying, which matters for anaesthesia.

If you have type 2 diabetes, speak to the doctor or nurse who looks after your diabetes before you stop Wegovy: the NHS says stopping could affect your blood sugar.

If you feel unwell after stopping, or a side effect does not settle, message your pharmacist through your account. Messages are not for emergencies. For urgent medical concerns, call NHS 111 or 999.

    Frequently asked questions

Yes, you can stop Wegovy at any time, but speak to your pharmacist or doctor first. The NHS advises talking to your doctor before you stop semaglutide and not stopping suddenly, and says a doctor may suggest reducing the dose gradually. If you have type 2 diabetes, stopping could affect your blood sugar, so speak to whoever looks after your diabetes as well. If a side effect is the reason, there may be other options, such as going back to the previous dose until it settles.

The UK product information for Wegovy sets no fixed length of treatment. It says a decision is needed on whether to continue if you have not lost at least 5% of your starting weight after 6 months on treatment. Through Solira, a GPhC-registered pharmacist reviews you regularly.

Semaglutide, the medicine in Wegovy, leaves the body slowly: the UK product information says it is present in the circulation for about 7 weeks after the last 2.4mg dose. As it goes, its effect on appetite wears off and weight usually starts to come back. In the STEP 1 trial extension (Diabetes, Obesity and Metabolism, 2022), adults who had completed 68 weeks of semaglutide 2.4mg had lost 17.3% of their weight on average, then regained 11.6 percentage points in the year after stopping, ending 5.6% below their starting weight. Individual results vary.

On average, not within the year that was studied, though weight regain is common. In the STEP 1 trial extension (Diabetes, Obesity and Metabolism, 2022), adults who had completed 68 weeks of semaglutide 2.4mg and lost 17.3% of their weight on average regained 11.6 percentage points in the year after stopping, ending 5.6% below their starting weight, against 0.1% below for those who had taken placebo. That year included no structured diet and activity programme. Individual results vary.

The UK product information for Wegovy pens and Wegovy tablets does not list any withdrawal symptoms. What people usually notice is their appetite returning as semaglutide leaves the body, over about 7 weeks after a last 2.4mg injection or about 5 to 7 weeks after a last 25mg tablet. If you have type 2 diabetes, the NHS says stopping could affect your blood sugar. If you feel unwell after stopping, speak to your pharmacist or GP.

Yes, you can stop at any time, but talk to your pharmacist first. After 1 month you would have had only the 0.25mg starting doses, which are built up slowly to make stomach side effects less likely, so it is early to judge whether Wegovy works for you. The UK product information looks at progress after 6 months, when a decision is needed on whether to continue if you have not lost at least 5% of your starting weight. If side effects are the reason, a slower dose increase may be possible.

Keep the eating and activity habits you built on treatment. Treatment works alongside a reduced-calorie diet and regular activity. Aim for at least 150 minutes a week. Weight loss naturally slows over time, and stopping treatment without keeping lifestyle changes commonly leads to weight regain, so we review you regularly and plan any stop together. The NHS physical activity guidelines also advise strengthening activities that work all the major muscles on at least 2 days a week, and our guide to eating on Wegovy suggests some protein at every meal. Weighing yourself regularly helps you notice a change early.

Yes. If your last dose was under three months ago, you can usually carry on at the same dose once your pharmacist has checked it (we ask for proof if your last supply came from another provider). After a break of three months or more, you restart at the lowest dose, 0.25mg for the pens or 1.5mg for the tablets, and build up again, with your pharmacist reviewing you before each step.
