Oral semaglutide is a small molecule wrapped in an absorption enhancer, but it is still genuinely difficult for the body to absorb. How you take the tablet matters as much as the tablet itself. Get the routine right and it works. Get it wrong and much of the dose passes straight through. This is the pharmacist's guide to taking Wegovy tablets properly.
The core rule
- One tablet, once a day.
- First thing when you wake, after an overnight fast of at least about 8 hours.
- Swallow whole with no more than a sip of plain water — up to 120 mL.
- Do not split, crush or chew the tablet.
- Wait at least 30 minutes before eating, drinking anything else, or taking any other oral medicines.
Why the 30 minutes matters
Oral semaglutide's absorption is low and highly sensitive to whatever else is in the stomach. Food, tea, coffee, juice, milk, and other tablets all compete for the small absorption window and can significantly reduce how much of the dose reaches your bloodstream. Taking it too close to breakfast is the single most common reason the pill "doesn't seem to work". Treat the 30 minutes as non-negotiable.
The dose ladder
You do not start on the maintenance dose. Wegovy tablets are titrated slowly so your gut can adjust:
- 1.5mg — starting dose
- 4mg
- 9mg
- 25mg — maintenance
You stay at least a month at each step before moving up. Some people stop at 9mg if that is enough; others go all the way to 25mg. Escalation is a pharmacist-led decision based on how you're tolerating treatment and the progress you're making.
What to do if you miss a dose
Skip the missed dose and take your next tablet at the usual time the following morning. Do not double up and do not take an extra tablet later the same day to catch up — this raises the risk of nausea and vomiting without improving results.
Building the routine
The habits that make this work are boring but effective. Keep the tablet blister and a small glass on your bedside table so you take it before you get up. Set a 30-minute timer as soon as you swallow it, and use that time for the shower, dressing, dog walk — anything that doesn't involve tea, coffee or other tablets. Move any morning medicines (blood pressure, thyroxine, statins) to after the 30-minute window, unless your prescriber tells you otherwise.
Frequently asked questions
What if I eat too soon? Absorption drops significantly. Don't take another tablet — just carry on the following day and be stricter with the timing.
Can I take it at night? No. It's designed to be taken after an overnight fast, when the stomach is genuinely empty.
What if I miss a dose? Skip it and take the next dose the following morning. Never double up.
Can I take other medicines with it? Not within 30 minutes. Move them to after the window, or discuss the timing with your pharmacist.
When to see your pharmacist or doctor
- Severe or persistent vomiting, or signs of dehydration
- Severe abdominal pain (which can rarely signal pancreatitis)
- Any new medicine started by another prescriber — the timing may need reviewing
- Before increasing to the next dose step — escalation should always be pharmacist-led
