Wegovy tablets and Rybelsus contain the same active ingredient — oral semaglutide. It's easy to assume they're interchangeable. They are not. The molecule is identical, but the doses, the licensed use and what each product has actually been studied to do are different. Here's what that means in practice.

Same molecule, two products

Both Wegovy tablets and Rybelsus deliver semaglutide by mouth using the same absorption technology and the same empty-stomach 30-minute rule. Where they differ is what strengths they come in, and what they are licensed to treat.

Rybelsus

  • Doses: 3mg, 7mg, 14mg.
  • Licensed for type 2 diabetes only — to help control blood glucose.
  • Not licensed for weight management. Any weight change on Rybelsus is a secondary effect, not the reason it's prescribed.

Wegovy pill

  • Doses: 1.5mg, 4mg, 9mg, 25mg.
  • Licensed for weight management in adults living with obesity (BMI ≥30), or overweight (BMI ≥27) with a weight-related condition.
  • The 25mg maintenance dose is the version studied in OASIS-4, the trial that supports its licensed weight-loss outcome.

Why the distinction matters

Rybelsus tops out at 14mg. Wegovy tablets go up to 25mg. The evidence for meaningful weight loss — around 14% average at 64 weeks in OASIS-4 — was generated at 25mg, not at Rybelsus doses. Using Rybelsus for weight loss is off-label: the strength is lower than what was studied, and it isn't the product a UK prescriber should choose for weight management when a licensed weight-loss version now exists.

This isn't a technicality. Using the licensed product means the dose, monitoring and information you're given all match the indication you're being treated for. Using the diabetes version off-label doesn't.

What's the same

  • The same active ingredient — semaglutide.
  • The same absorption behaviour — both must be taken on an empty stomach with a sip of water, with a 30-minute wait before eating, drinking or other oral medicines.
  • Broadly the same side-effect profile (nausea, GI symptoms, mostly during dose changes).

Frequently asked questions

Are they the same drug? The active ingredient is the same. The product, the doses and the licensed use are different.

Can I use Rybelsus for weight loss? Rybelsus isn't licensed for weight loss. If weight management is the goal, the Wegovy pill is the appropriate licensed option.

Which is stronger? Wegovy tablets go up to 25mg; Rybelsus tops out at 14mg. The Wegovy pill's maintenance dose is considerably higher.

Do they have the same side effects? Broadly yes — same molecule, same GI-heavy profile, mostly during dose increases.
