What people mean by the golden dose
The "golden dose" — sometimes called the "fifth dose" — refers to the widely shared idea that a Mounjaro pen contains extra medicine beyond its labelled doses, and that a further injection can be extracted from it.
Why overfill exists
Injectable devices are manufactured with a small excess volume. This is there to guarantee that every labelled dose delivers a full, accurate amount, allowing for priming, dead space in the mechanism and normal manufacturing tolerances. It is not a bonus dose.
The honest position
The manufacturer specifies a fixed number of doses per pen. Any residual volume beyond that is not a guaranteed or verified dose. Attempting to extract it risks an inaccurate or incomplete dose, and repeated needle changes on a spent pen introduce a contamination risk. It also falls outside the licensed use of the device, which means neither the manufacturer nor your pharmacist can stand behind what you receive.

Use the labelled number of doses and dispose of the pen in your sharps bin.
